Mint Devs LLC (“we,” “us,” “our,” or “Mint Devs”), a limited liability company organized under the laws of Wyoming, United States, provides the Jitsu Forge mobile application and its cloud-based features (collectively, the “Service”), which is suitable for users of all ages, including children under 13 (or under 16 in the European Union, under 14 in China) with parental consent. This Parental Consent Form (“Form”) allows parents or legal guardians (“Parent,” “you,” or “your”) to provide verifiable consent for their child to use the Service, as required by the Children’s Online Privacy Protection Act (COPPA) in the United States, the General Data Protection Regulation (GDPR) in the European Union, China’s Personal Information Protection Law (PIPL), and other applicable laws. By completing and submitting this Form, you agree to the collection, use, and disclosure of your child’s personal information as described in our Privacy Policy, available in the app or at https://jitsuforge.com/legal/privacy-policy-en (main page only accessible from China).

I, [Parent’s Full Name], as the parent or legal guardian of [Child’s Full Name], hereby consent to Mint Devs LLC collecting, using, and disclosing my child’s personal information (e.g., email address, training logs, and other data necessary for the Service) as described in the Jitsu Forge Privacy Policy. I understand that:

My child’s personal information will be used to provide and improve the Service, such as creating and syncing Brazilian Jiu-Jitsu training plans.
Mint Devs will limit data collection to what is necessary for the Service and will not use my child’s data for marketing or targeted advertising.
My child’s data will be stored securely on servers in the Singapore, with enhanced protections as required by COPPA, GDPR, and PIPL, but no system is completely secure.
For users in China, my child’s data will be transferred to and stored in the United States, and I explicitly consent to this cross-border transfer as required by PIPL. I may withdraw this consent, which may limit Service functionality.
The Jitsu Forge website (jitsuforge.com) has only its main page accessible from China as a non-commercial landing page. All other pages, including payment and account pages, are inaccessible, and all payments for Chinese users are processed through app store in-app purchases.

4. Parental Rights

I understand that I may:

Review my child’s personal information by contacting Mint Devs at [email protected].
Request modification or deletion of my child’s data.
Revoke this consent at any time, which will result in the deletion of my child’s account and personal information, subject to legal requirements.
Access the Privacy Policy and, for Chinese users, the PIPL Addendum in the app or at https://jitsuforge.com/legal/privacy-policy-en and https://jitsuforge.com/legal/pipl-en (main page only accessible from China) for details on data practices.

5. Subscriptions and Purchases

I understand that my child may access premium features (e.g., enhanced cloud sync, premium training plans) through subscriptions or one-time purchases, which require my additional consent and payment verification. I agree to:

Authorize my child to access premium features via subscriptions or one-time purchases (check if applicable).
Provide payment information (e.g., credit card) for verification, if required, to enable such purchases.
Manage any subscriptions or purchases through the applicable app store (e.g., Apple App Store, Google Play, Huawei AppGallery) for all users, including those in China.

6. Verification Method

To verify my identity as the parent or legal guardian, I will complete one of the following (select one):

Email Confirmation: I will respond to a confirmation email sent to [Parent’s Email Address] to verify my consent.
Payment Verification: I will provide a small, refundable payment (e.g., $0.50) via credit card to verify my identity, as prompted during account creation or purchase in the app.
Signed Form: I will print, sign, and return this Form via email to [email protected] or upload it in the app.

I acknowledge that I have read and understood the Jitsu Forge Privacy Policy and, for Chinese users, the PIPL Addendum, and agree to the terms of this Form. I confirm that I am the parent or legal guardian of the child named above and have the authority to provide this consent.
